El Hogar's Sierra Elder Wellness Program (SEWP) is a specialized field program dedicated to the mental health needs of older adults (55+) in Sacramento County. The program provides comprehensive outpatient services, including psychiatric medication management, therapy, and case management, designed to support seniors experiencing functional impairment due to chronic mental health symptoms. Uniquely, SEWP operates with no cost to the client, funded entirely by the Sacramento County Division of Behavioral Health Services.
