About Upham's Home Health Care
Upham's Home Health Care/Visiting Nurse program has been serving Boston residents since 1975 as the only home health and visiting nurse program offered by a Boston health center. The program enables patients to return home sooner, remain in their homes longer, and prevent hospital re-admissions through individualized treatment plans developed collaboratively with patients and care teams. Upham's Corner Health Center provides comprehensive home-based skilled nursing and support services to help seniors recover safely in the comfort of their own homes.
